Obstetrics/Gynecology PIFs

Effective July 1, 2007, the Competency Assessment Form will no longer be used.

Beginning August 1, 2006, program directors should not attach the program level letters of agreement to the PIF for a continued accreditation review. All letters of agreement must be available onsite during the site visit. In addition, letters of agreement must be included in the documents submitted for an application for a new specialty or subspecialty program.

Obstetrics/Gynecology			New Application Only (7/2008) Programs scheduled for a site visit before December 21, 2008 should complete this PIF Continued Accreditation (01/31/2008) Programs scheduled for a site visit after January 1, 2009, should access the PIF through the Accreditation Data System (ADS) using your secured login and password.
